POSITION SUMMARY:This role is a hybrid role consisting of both the Chronic Care Case Management and Annual Wellness nurse duties. Chronic Care Case Management: The Chronic Care Case Manager utilizes a collaborative process of assessment planning facilitation and advocacy for options and services to meet Medicare patients health needs through communication and targeted intervention to promote optimal cost-effective outcomes in a clinic setting. The function of this position is to assist Medicare patients in the administration of their health plan benefits. The Chronic Care Case Manager focuses on arranging or coordinating services that the patient needs to get well or stay well and removing barriers that prevent the member from engaging in an appropriate plan of care. The Chronic Care Case Manager demonstrates strengths working independently as well as collaboratively within a multi-specialty healthcare Chronic Care Case Manager may rotate among several clinics performing non-face-to-face encounters with the clinics Medicare population for that specific area. The Chronic Care Case Manager performs duties and responsibilities in a manner consistent with the Mission and Values of Hattiesburg Wellness Nurse: The Annual Wellness nurse is responsible for performing annual Medicare examinations for Medicare and Medicare fee-for-service patients in a clinic setting. Wellness exams for these patients are recommended Wellness Nurse may rotate among several clinics performing wellness exams. The Wellness Nurse must be flexible and be willing to help in other areas of the clinic if the need arises. The Wellness Nurse performs duties and responsibilities in a manner consistent with the Mission and Values of Hattiesburg QUALIFICATIONS: Graduate of accredited school of nursing required-Registered Nurse with current state licensure required. Prefer at least three years of EMR medical management and policies and procedures. Intermediate skills in work processing spreadsheet and database development and maintenance. Excellent organizational written and oral communication SCHEDULE: Monday-Friday 8:00am-5:00pm
