POSITION SUMMARY:The Accountable Care Organization (ACO) Case Management Nurse is responsible for reviewing patient medical records to identify preventative care measures and screenings such as mammograms colonoscopies immunizations medication adherence and more. Once these needs are identified the ACO nurse coordinates and schedules the appropriate preventive services and wellness role requires flexibility as the nurse may be called upon to assist in other areas of the clinic as needed. Duties must be performed in a manner consistent with the Mission and Values of Hattiesburg & EXPERIENCE:Graduation certificate from an accredited RN school of nursing requiredCurrent state licensure requiredBLS/CPR certificationQUALIFICATIONS:Strong ability to maintain strict confidentiality and handle sensitive information with discretionHighly organized and detail-oriented especially in time managementAbility to prioritize key objectives while managing multiple tasks simultaneouslyExcellent written and verbal communication skillsEffective interpersonal negotiation and conflict resolution abilitiesProficiency with Electronic Medical Records (EMR) medical management systems and knowledge of related clinical policies and proceduresPHYSICAL REQUIREMENTS:Ability to stand and walk for up to 8 hours per dayAbility to bend stoop twist and assist patients with movement and positioningFrequent keyboarding using a traditional keyboardOccasional lifting carrying pushing pulling or reaching (overhead and at/below shoulder level)Constant communication (speaking and hearing) in-person and via telephoneWORK SCHEDULE:Monday- Friday (Hybrid)
