Position Highlights
MondayFriday 8:00 AM5:00 PM
Shared call rotation
In-person and virtual visits supported by Medical Assistant
Manage complex patients with ongoing follow-up throughout stay
Collaborate with SNF team to facilitate care coordination
Implement discharge planning to improve patient outcomes
Signing bonus CME and relocation available.
Candidate Qualifications
Ideal Candidate
Physician Assistant or Nurse Practitioner
Passion for working in a SNF/Post-Acute Care Environment
Experience in a Skilled Nursing Facility is preferred but not required
