Due to our continued commitment in supporting people suffering with substance misuse problems Change Grow Live are looking for a Recovery Coordinator to join our busy Birmingham service working in the Heartlands hospital as our Specialist In-Hospital Liaison.

Where:Heartlands Hospital Birmingham

Hours: Full Time 37.5hrs per week

Full-Time Salary Range: 27861.26 - 32002.35 (Based on full time hours pro rata for part time hours)

* Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role:

  • Set up effective communication and referral pathways with hospital-based specialist teams acting as a point of contact for acute referrals
  • Participate with acute services to create/maintain a process for targeting repeat attendees implementing an investigation system that enables wider services to critically evaluate the circumstances around an individuals hospital admission
  • Provide support to acute hospital services to avoid unnecessary admission to hospital of people who can be supported and treated at community addiction services
  • Develop specific links with identified specialist acute services and provide in reach to hospital wards including Gastroenterology/Hepatology; specialist clinics observation ward; A&E
  • Provide advanced assessment planning implementation and evaluation of care to a small complex specialist caseload of patients who misuse alcohol and/or drugs while they remain in a hospital setting or as part of the discharge to community plan
  • Work with community teams to ensure assertive and effective follow up of both repeat presentations and discharges from the hospital
  • Provide support and advice to ward staff of those identified as misusing alcohol and/or drugs to prevent further complications from their conditions both in the short term and the long term

About you:

  • Have a strong understanding of substance misuse issues and experience of working within a similar field
  • Be an excellent communicator both verbal and written with good IT skills
  • Proactive and able to work on own initiative with excellent time management and prioritising skills
  • Have a good working knowledge of mental health interventions services and good practice
  • Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals agencies and internal/external stakeholders
  • Ability to manage change successfully in a way that prioritises the needs of service users
  • Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
