Role Scope:

This is an opportunity for a mission-drivenleader who is passionate aboutsupporting ourPACE participantswith their social workneeds andadvocating for what matters most toeach individual. TheSocial Work and Advocacy Supervisorat the PACE Center oversees the daily operations ofourSocial Work team Participant Advocates and any behavioral health are relentless in understanding the needs of our participantsadvocating forour IDT to keep those needs at the forefront of each interaction and seekingeffective ways tomeet those Work and Advocacy Supervisorprovidesleadershiptotheir direct reportswhilealsocollaborating with fellow leaders to ensureparticipants physical socialemotionaland recreational needsare position ensures high-quality person-centered care in alignment with PACE standards and regulatory guidelines.

• Contribute to acenterexperience that Participants want to spend time in a team culture that cares and creates joy and an environment where all participants and team members belong.

• Continue to raise the sharefeedback andhelp us implement changesin order toimprove clinical outcomes and experience for participants.

• Exhibit and honor Habitats values.

• Supervise one or moreSocial Workersensuringhigh quality social services delivered to our participants.

• Oversee ourNew Participant Advocate(s) ensuringa strong welcome into our PACE program and a focus on building love and trust during participants transition into the program.

• Serve as a primary liaison with community and government resources on behalf of the PACE program.

• Serve asescalationresource for participant issues (grievances behavioral social work engagement).

• Support team in care planning and driving resolution of complex family dynamics / social work needs.

• Develop andmaintainstaffing schedules andassignment ofduties.

• Collaborate with the interdisciplinary team (IDT) to assess participant needs and update care plans.

• Support a culture of safetydignityand respect for all PACE participantsand staff.

• Performs related duties as assigned.

Required Qualifications:

• Masters in Social Work.

• Knowledge ofPACEprogramor similar healthcare operations.

• Knowledge of socialwork-relatedneeds of frail older adults as well as knowledge of community and government resources to meet those needs.

• Ability to be self-directed and able to work independently.

• A minimum of two yearsin a supervisory position.

• Proof of medical clearance for communicable diseases including a TB test.

• Proof of all immunizationsareup to date.

• Proof of current CPR/BLS certification required or requirement to obtain within30 daysofemployment.

• Proof of valid CA drivers license personal transportationgood drivingrecordand auto insurance as required by State law.

Preferred Qualifications:

• Licensed Clinical Social Worker (LCSW).

• Bilingual: Spanish preferred.

• A state issued drivers license personal transportation and auto insurance as required by law.

Compensation:

We take into account an individuals qualifications skillset and experience in determining final salary. This role is eligible for medical/dental/vision insurance short and long-term disability life insurance flexible spending accounts 401(k) savings paid time off and company-paid holidays. The expected salary range for this position is $90000 - $116000and is bonus actual offer will be at the companys sole discretion anddeterminedby relevant business considerations including the final candidates qualifications years of experience skillset and geographic location.
