Amity Health is seeking a dedicated ITC Care Coordinator to deliver culturally appropriate care coordination services to Aboriginal and Torres Strait Islander people living with chronic health conditions.
In this role youll work closely with General Practitioners Aboriginal Health Workers and other health professionals to improve access to primary care strengthen continuity of care and support improved health outcomes. Youll provide proactive coordination across the care continuum support access to supplementary services and contribute to building the cultural capability of mainstream primary health providers.
