Karoo is seeking to add an important care team member the Patient Engagement Specialist. This individual will be responsible for new patient enrollment and play a critical role in driving growth within the organization. This role integrates within partner cardiology practices and will focus on screening eligible patients for enrollment communicating with practice providers to discuss eligibility interacting with prospective patients to explain services address inquiries gather information provide education and schedule initial appointments with the virtual team.

This role requires someone who is comfortable being the sole representative of Karoo on-site at partner locations and who can be trusted to make day-to-day decisions that reflect the companys values and standards.

Responsibilities

• Maintain daily on-site presence at partner cardiology practices.
• Support additional clinics remotely as needed.
• Pre-screen patients for eligibility and notify practice providers of prospective enrollments based on patient risk profiles.
• Verify patient program eligibility and elicit feedback from practice providers on enrollment potential.
• Conduct enrollment onboarding sessions with patients following a referral from a partner practitioner.
• Maintain documentation in Karoos care management system and the partner cardiology practice EMR for tracking and reporting purposes.
• Drive patient enrollment by providing an overview of the Karoo Health Care Program through patient education and interaction to ensure patients understand program services and benefits.
• Facilitate assignment and transfer to the virtual team for continued services.
• Coordinate with the partner cardiology practice to provide insight into the patients care journey.
• Regularly interact with practice staff to provide line-of-sight into patient admissions readmissions and ED visits.
• Manage inbound calls from patients responding to inquiries about enrollment and ongoing engagement with the Karoo Care Program.
• Implement new strategies to grow appropriate enrollment into the Karoo Care Program.
• Provide educational presentations to practice partners regarding the Karoo Health program.
• Communicate regularly with all members of the interdisciplinary team to share key information coordinate tasks and provide support for the enrollment process.
• Adhere to the cardiology practices clinical guidelines and protocols with oversight from the clinics Practice Administrator to ensure seamless alignment with clinic operations and standards of care.
• Special projects initiatives and other job duties as assigned.

Qualifications

• Minimum 3 years of experience working independently in a healthcare clinical or physician-practice environment with direct patient and provider interaction.
• Demonstrated ability to operate autonomously in a clinic setting manage competing priorities and make sound judgment calls without direct on-site Karoo Health supervision.
• Proven track record of representing an organization professionally with physicians practice leadership and patients in high-trust high-stakes environments.
• Strong written and verbal communication skills with the ability to clearly explain programs workflows and next steps to both clinical and non-clinical audiences.
• High level of professional presence reliability and follow-through; consistently prepared punctual and accountable.
• Comfortable navigating ambiguity in a fast-paced growing organization and translating evolving guidance into effective action.
• Ability to quickly learn and competently use multiple technology platforms (EHRs enrollment systems care management tools).
• Experience managing sensitive patient information with discretion and sound judgment.
• Ability to identify issues proactively escalate appropriately and propose solutions rather than waiting for direction.
• Strong organizational skills and attention to detail particularly in documentation eligibility verification and handoffs to care teams.
• Healthcare certification (CMA CHW CPhT RHIT etc.) or equivalent hands-on experience.

Preferred Qualifications

• Prior experience embedded within specialty practices (cardiology strongly preferred).
• Experience in patient enrollment care coordination or value-based care programs.
• Comfort leading small group trainings or educational sessions with clinic staff.
