BASIC FUNCTION
The Health and Wellness Supervisor provides service to the residents in compliance with appropriate codes including emergency medical needs and coordinating physical and psychosocial needs of the resident with emphasis on promoting wellness and independence. They also provide overall supervision of Resident Assistants in Health & Wellness (RCAC) as well as provide back-up emergency service to other residents of Oakwood Village as needed/requested.

2. Responds to resident emergencies collecting data regarding change of condition in mental or physical conditions provides services to residents that meet their physical emotional intellectual social and spiritual needs and reports these changes to the Director/RN/Physician Family HCPOA.
3. Participates in discussion and development of residents individual care plan/ISP. Maintains the residents records and charts. Updates ISP on an ongoing basis.
4. Collaborates with Social Worker HRC AL Director and Sales and Marketing as resident may need to be transferred to a more appropriate level of care or setting due to high care needs.
5. Assist with resolution of an ongoing resident/family/staff concern. Notifies Director if needed.
6. Provides back up to the resident assistants in providing scheduled and unscheduled care to the residents.
7. Provides direct supervision & leadership to all Resident Assistant Staff in Health & Wellness promoting teamwork a positive work environment and following the policies of Oakwood Village.
8. Supervises Resident Assistants in passing and recording the administration of scheduled and PRN medications and treatments as needed. Reports medication errors to the Director/RN and completes the necessary paperwork.
9. Assures that Resident Assistants are familiar with and providing services/support as indicated on each residents care plan/ISP.
10. Provides oversight and participates in the orientation of the new Resident Assistants training ensuring the new employees training is completed satisfactorily and the new employee verbalizes understanding of job duties and departmental expectations
11. Obtains and transcribes physicians orders as needed. Communicates the orders the Pharmacy. Reports any need for follow-up to oncoming staff. Communicates changes to resident and HCPOA as needed.
12. Reports medication errors falls change of condition to physicians as they occur and communicates this to the Director/RN family HCPOA and fills out necessary paperwork.
13. Attends appropriate staff development sessions for development and growth.
14. Attends staff meetings and other meetings as assigned.
15. Works with Scheduler and does scheduling of staff as assigned.
16. Motivates and encourages staff to reach their optimal performance level.
17. Attends Interdisciplinary Team (IDT)Staffing Supervisory and any other meetings as scheduled
18. Assist with necessary documentation related to billing.
19. Assists with interviews of potential new staff.
20. Participates in all staffing functions including hiring counseling discipline and performance reviews.
21. Oversight of day to day operations of Health & Wellness Department.
22. Performs other duties as needed.
QUALIFICATIONS
1. Possess a genuine caring and ability to relate well to the residents.
2. Knowledge and skill in current LPN/RA practices especially relating to geriatric population.
3. Ability to plan organize and prioritize workload.
4. Ability to build and maintain teamwork relationships with other staff.
5. Ability to effectively lead and supervise the Resident Assistants.
6. Ability to perform job responsibilities in a professional accurate and conscientious manner.
7. Ability to maintain and protect the confidentiality of information.
8. Ability to exercise independent judgment and make sound decisions.
9. Ability to provide leadership that fosters effective teamwork in meeting the mission of Oakwood.
10. Willingness to work on call weekends PMs/NOCs holidays as delegated by on-call schedules.
11. Knowledge of all appropriate codes.
EXPERIENCE EDUCATION AND/OR TRAINING
1. Licensed as an RN in the State of Wisconsin.
2. 2 years of prior experience of RN and Supervisory preferred.
3. CPR certified and CBRF certified preferred.
