What Youll Do:

In this dynamic role you will work closely with youth in our HEARTH Beyond Program and provide support at My Sister Susans House our residential facility for youth. Your main responsibility will be to create and implement individualized case management plans to ensure successful outcomes for our clients. You will engage with clients face-to-face coordinate evaluations and collaborate with them to develop client-centered plans focused on health wellness and personal development.

Qualifications

Education and Experience

Minimum age of 21 years.
Bachelors degree from a four-year college or university with at least two years experience providing case management or equivalent combination of education and experience.
Proficient in reading and interpreting documents as well as writing routine reports and correspondence.
Basic math skills including multiplication division understanding units of measure and using whole numbers fractions and decimals.
Ability to follow written oral or diagram form instructions with common sense understanding.
Valid drivers license at least three years of driving experience and a driving record in good standing.
