University Home Care Inc is seeking a compassionate and skilled Contingent Hospice Registered Nurse to join our team in Livonia MI. This role involves providing exceptional end-of-life care to patients and supporting their families with empathy and professionalism. The ideal candidate will have a strong background in hospice nursing and be committed to delivering patient-centered care in a home setting.
Responsibilities
- Provide skilled nursing care to hospice patients in their homes ensuring comfort and dignity.
- Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team.
- Monitor patient condition document observations and report changes accurately to the care team.
- Educate patients and families about hospice care symptom management and medications.
- Collaborate with physicians social workers and other healthcare professionals to coordinate care.
- Assist with pain and symptom management including medication administration as prescribed.
- Maintain compliance with all regulatory and organizational policies and standards.
Requirements
- Valid Registered Nurse (RN) license in the state of Michigan.
- Previous hospice or palliative care nursing experience preferred.
- Strong clinical assessment and patient care skills.
- Ability to work independently and manage time effectively in a home care setting.
- Excellent communication and interpersonal skills.
- Current CPR certification.
- Reliable transportation to visit patients at their homes in Livonia MI.
