Job Summary

The Manager Care Management determines care management strategy and leads the care management team in pursuit of market goals. They ensure compliance with program requirements by liaising with contract and program-specific teams and Aledade. They coach and support partner practices in understanding and succeeding within these programs provide thought leadership on care management program roles and metrics manage care team members and develop and deploy training programs. Additionally they design and operationalize vendor contracts coordinate interdisciplinary care management interventions and stay updated on industry intervention evidence and adjust Aledade services accordingly.

Lead transformative care management initiatives as our Manager Care Management who will directly improve patient outcomes across their given market. Their leadership will span from developing evidence-based training programs and managing vendor partnerships to providing thought leadership on care management metrics and coordinating life-changing interventions for complex patients. Please note: Candidates will need a Registered Dietician Nurse LMSW or LCSM Licensure in the state of Michigan to be considered.

Primary Duties:

    • Manage from end-to-end a Care Management program that incorporates Aledade perspective on effective care management interventions. Leverage internal and external experts and literature. Utilize internal staffing as well as vendor contracts to execute programs and be accountable for the quality and impact of the Aledade Care Management program as well as the tools used to coordinate internally. Additionally design negotiate and operationalize contractual arrangements with vendors that are hired to support the goals of the supported programs pulling in diversity of thought and perspective from others when needed.
    • Stay up to date with program and regulatory requirements applicable to the Care Management program and implement changes to ensure ongoing program and regulatory compliance.
    • Directly manage coach and lead a team of interdisciplinary care managers (and community health workers where relevant) ensuring patients receive the appropriate care at the appropriate time to help prevent unnecessary medical spend and promote high quality of care. Develop metrics for direct reports to measure their success & the success of the overall program. Determine and deploy appropriate staffing assignments considering team skillsets and practice partner attributes and preferences as well as the strategic goals of the market. Ensure team members including junior team members are performing top of license scope of work.
    • Provide physician and administrative contacts in partner practices with regular updates performance metrics and ongoing guidance related to the services they are receiving from the Aledade team incorporating asks and preferences of the physicians and administrators into the teams approach to the work.

Minimum Qualifications:

    • Registered Dietician Nurse LMSW or LCSM Licensure in the state of Michigan
    • Bachelors degree or equivalent.
    • Minimum 5 years experience in a care manager case manager or related role with at least 3 years in a people manager role.
    • At least 2 years experience providing care to complex high risk older and/or adults with disabilities.
    • Comprehensive knowledge of Social Determinants of Health (SDOH) including screening techniques and appropriate resources and interventions.
    • Technical mastery of electronic health record charting population health tools and health information exchanges. Experience designing and utilizing interdisciplinary team communications and protocols.
    • Experience managing projects and vendor relationships.

Preferred KSAs:

    • Care management credential(s) or certification(s) preferred

Physical Requirements:

    • Ability to travel to visit practices HQ or other team or company events as needed up to 4 times per month.
    • Sitting for prolonged periods of time. Extensive use of computers and keyboard. Occasional walking and lifting may be required.
