Job Summary

We are seeking a dynamic client focused Registered Nurse to join our multidisciplinary Allied Health Team. You will be working with a variety of clients across the aged and disability spectrum providing appropriate community-based care.

The Position

In this role you will work collaboratively with the clinical and multidisciplinary teams to support the effective delivery of primary health services. This includes conducting health checks providing client education and managing followup and recall. You will also participate in case conferences and coordinate care with internal teams and external service providers to ensure smooth referral pathways and integrated client support.

You will deliver highquality nursing care within Community Services programs including the Transitional Care Program (TCP) and Home Care and administer medications prescribed by external GPs while on TCP. Your clinical tasks may include pathology collection screening procedures ECGs spirometry wound care immunisations and assisting with minor procedures such as plastering and iron infusions.

About you:

You will be a passionate Registered Nurse who is looking to work within a Not For Profit that directly supports the local Aboriginal families and community. You will also have the following:

  • Registered Nurse with current registration with AHPRA.
  • Demonstrated ability to plan develop implement and evaluate nursing care programs in a primary health setting
  • Demonstrated ability to communicate effectively and sensitively with Aboriginal people
  • A demonstrated understanding of the unique issues effecting and impacting upon the health of Aboriginal people
  • Demonstrated organisational and time management skills along with an ability to adapt to changing needs

It would also be beneficial but not essential to have the following experience:

  • Registered Nurse with at least 3 years post-graduation experience in nursing.
  • Experience in Community or Aboriginal Health
  • Knowledge and expertise in the use of Communicare or similar clinical database system.
