Overview

Diversicare of Pell City would like to invite you to join us for a Walk Up Interview Event and Job Fair from 7am 6pm on Wednesday March 4th 2026.

We will be conducting interviews and hiring on the same day qualified applicants.

We will be hiring for these positions:

  • RN
  • LPN
  • CNA
  • Certified Med Tech

Please bring the following items with you:

1. Resume (If you are unable to bring a resume you can complete an application onsite to summarize your work history and qualifications.)

2. Drivers License or State ID

3. Social Security Card (If you do not currently have a social security card we will help you order a new one online.)

4. Direct Deposit Information (Pay cards are available for those that do not want to sign up for direct deposit.)

Responsibilities

  1. Assists in pre-admission and admission process with customers; aids clients in adjusting to nursing home placement.

  2. Interviews residents and families to obtain relevant past and present adjustments behavior and lifestyle.

  3. Documents progress and updates plans of care at the interdisciplinary care conference.

  4. Arranges transportation makes appointments and acts as a liaison between all departments residents families and outside agencies.

  5. Assists in handling and accepting placement through one-on-one counseling and group work.

  6. Ascertains potential and develops discharge plans when clients areadmitted.

  7. Arranges for post discharge services and follow-up care.

  8. Assesses emotional financial spiritual educational and social needs of residents; ensures those needs are Manager Education & Training by the facility or through the use of outside agencies.

  9. Acts as resident/family advocate and ensure the resident is knowledgeablein and exercises his/her rights.

  10. Responsible for organizing and conduction of Family Counsel Meetings and maintaining minutes of the meeting.

  11. Maintains effective communication with residents families healthcare professionals and facility staff.

Qualifications

  1. Bachelors or Masters Degree in Social Work
  2. License with State

  3. One year of supervised social work experiencein a health care setting working directly with patients and families.

  4. Experience in working in healthcare setting preference in nursing home hospice home health hospital.

  5. Ability to work with a variety of people in situations varying from community and nursing home planning.

  6. Able to make decisions concerning procedures based on evaluations and interpretations of case histories.
