General Summary: The Hospital Dedicated Discharge Planner is responsible for outreach to members who meet wrap-around services criteria and high utilization of hospitals and level 1 facilities. The Hospital Discharge Planner will facilitate successful transition from a Level I or Crisis Facility to outpatient services or other appropriate level of care and promote recovery skill building and utilization of community/natural RESPONSIBILITIES:Successfully orient members to wrap-around services and outpatient/step-down transportation assistance with hospital diversions and discharges as intensive outreach and case management services to members to avoid gaps in services and to provide continuity of care (including but not limited to diversion educational activities med cueing emergency case management environmental assessments transportation referrals ART meeting etc.).Helps enrolled members develop community and natural supports and development of community/natural support members in implementation of Crisis/Safety openly and effectively with treatment with Outpatient staff Hospital staff Level I staff and case management team staffs to facilitate transitional services crisis intervention and alternative plans as progress daily discuss issues with Supervisor under direction of Case Management Supervisor and coordinate interventions as community resources and existing CPI support services to provide additional supports to for members as needed to ensure delivery of necessary -up with member as needed to facilitate appropriate linkages until client is stable and ready for transition to standard case all appropriate records related to job comprehensive assessments and service plans. Completes an appropriate clinical staffing communication for all services rendered during business with Crisis Response Network staff as ongoing on-call crisis support for members phoning the after-hours other related activities in accordance with agency growth and changes. All clinical staff will participate in the rotation of on-call duties by the carrying of the company on-call cell phone. This duty includes overnight weekends and for following any policies procedures and controls established by the organization the HIPAA Privacy Officer and/or the HIPAA Security Officer regarding access to protection of and the use of the PHI. Performs other related duties as :Education Masters degree in a social work psychology counseling or related behavioral health field; or Bachelors degree in social work psychology counseling or related behavioral health At least three (3) year of experience working in the public behavioral health system (preferably with members with SMI)REGULATORY:Minimum 21 years of Level I fingerprint clearance (must possess upon hire and maintain throughout employment).CPR First Aid AED certification if required (must possess upon hire and maintain throughout employment).Current valid Arizona Drivers License and 39-month Motor Vehicle Report and proof of vehicle registration liability insurance to meet insurance requirements if current negative TB test result if required (Employer provides).Questions about this positionContact us
