We are seeking a highly organized and professional Remote Referral Intake & Qualification Specialist to support our Healthcare Referral Liaison in Collier County Florida.
This role is responsible for:
Calling hospitals rehab centers and nursing facilities
Identifying discharge planners and social workers
Introducing our 24/7 care services
Pre-qualifying potential referrals
Scheduling appointments for our US-based Liaison
This is NOT a hard sales role.
This is relationship-building and qualification.
Core Responsibilities:
Outbound Outreach
Call hospitals and rehab facilities daily
Identify decision-makers (discharge planners case managers)
Introduce our services briefly and professionally
Referral Qualification
Ask structured questions:
Is the patient discharging within 2472 hours
Does the patient need home care
Is care needed 24/7 live-in or part-time
Is the patient private pay or insurance-based
Appointment Setting
Schedule in-person visits for the Healthcare Liaison
Confirm time location and contact person
Send confirmation email (template provided)
CRM Management
Document every call
Track referrals
Update pipeline daily
Maintain clean data
Daily KPIs (Important)
4060 outbound calls per day
510 meaningful conversations
35 scheduled introductions weekly
100% CRM documentation
Requirements
Required Skills:
Clear English speaking (neutral accent preferred)
Professional phone etiquette
Healthcare experience preferred but not required
Comfortable calling US medical facilities
Organized and detail-oriented
Able to work US Eastern Time hours
Benefits
Base: $600$900/month
Required Skills:
Assist patients with activities of daily living (ADLs) including bathing dressing grooming toileting feeding and mobility support using assistive devices such as Hoyer lifts and transfer equipment Measure monitor and accurately record vital signs (blood pressure pulse temperature respiration) in electronic health record (EHR) systems such as Epic Cerner Meditech or eClinicalWorks Support licensed nursing staff with patient observation rounding and routine clinical tasks while adhering to HIPAA and facility protocols Provide compassionate care for patients with dementia Alzheimers disease behavioral health needs or post-acute rehabilitation requirements Collaborate with nurses physicians and interdisciplinary care teams to deliver coordinated patient-centered care Maintain infection control standards including aseptic techniques during wound care catheter care and isolation procedures Assist with patient transfers ambulation and positioning while ensuring safety and proper body mechanics Support rehabilitation and recovery programs by assisting patients with exercises mobility routines and daily activities Deliver respectful and compassionate care to diverse patient populations primarily adult and geriatric patients Document all patient care activities thoroughly and accurately within facility-approved medical documentation systems Experience & Qualifications Current and valid New York State CNA certification Previous experience as a CNA in hospitals long-term care facilities rehabilitation centers assisted living communities or similar healthcare settings preferred Experience in acute care medical-surgical units or high-acuity environments is a plus Familiarity with EMR/EHR systems such as Epic Cerner Meditech eClinicalWorks or similar platforms Knowledge of infection control practices patient safety standards and basic medical terminology Ability to work effectively within multidisciplinary healthcare teams Strong communication skills reliability and attention to detail BLS certification preferred
