Position Overview

An established home care agency serving seniors throughout Sedgwick County is seeking multiple Registered Nurses to perform in home field visits. We are hiring in specific communities including Derby Wichita Mulvane Haysville Maize and Colwich in order to match nurses with clients close to where they live and minimize drive time.

This role is ideal for an RN who prefers flexible scheduling and meaningful one on one patient interaction without hospital shift demands.

The RN will complete skilled nursing visits in private homes focusing on assessments care planning and oversight of services for elderly clients receiving home care support.

Key Responsibilities

• Conduct initial and ongoing nursing assessments in clients homes

• Perform health evaluations and document findings accurately

• Develop and update individualized care plans

• Complete supervisory visits for home health aides and caregivers as required

• Monitor chronic conditions such as diabetes hypertension COPD and heart disease

• Perform medication reconciliation and education

• Provide wound assessments and basic wound care as needed

• Complete post hospitalization follow up visits

• Communicate changes in condition to physicians and care team

• Ensure compliance with Kansas home care regulations and agency standards

• Maintain timely and accurate electronic documentation

Common Visit Types

• Start of care assessments

• 60 day supervisory visits

• Wellness and condition monitoring visits

• Medication setup oversight

• Safety evaluations and fall risk assessments

• Care plan updates

• Client and family education

Qualifications

• Active Registered Nurse license in Kansas

• Minimum one year of nursing experience preferred

• Home care hospice or case management experience strongly preferred

• Strong assessment and documentation skills

• Reliable transportation and valid driver license

• Comfortable working independently in the field

• Proficiency with electronic documentation systems
