Job Summary
Job Title: Occupational Therapist (OT) Skilled Nursing / Long-Term Care
Job Type: Contract
Duration: 13 Weeks
Urgency: Quick Start
Schedule: Monday Friday
Guaranteed Hours: 36 Hours per Week
We are seeking a licensed Occupational Therapist (OT) for a 13-week contract assignment in a Skilled Nursing Facility / Long-Term Care setting. The OT will be responsible for evaluating residents developing individualized treatment plans and providing therapeutic interventions to improve independence safety and quality of life.
Key Responsibilities-
Evaluate patients functional abilities related to activities of daily living (ADLs)

Develop and implement individualized occupational therapy treatment plans

Provide therapeutic interventions to improve strength coordination mobility and cognitive function
-
Train patients in the use of adaptive equipment and techniques
-
Monitor patient progress and modify treatment plans as appropriate

Maintain accurate and timely clinical documentation

Collaborate with interdisciplinary healthcare teams to support patient-centered goals

Educate patients families and caregivers on therapy strategies and safety

Active State Occupational Therapist license (must be current)

Minimum of 1 year of experience as an Occupational Therapist

Experience in Skilled Nursing Facility or Long-Term Care settings preferred

Strong clinical assessment communication and documentation skills
