Hospice RN Case Manager
Location: Baker City OR
Compensation: $32.00 $42.00 / Hour

Job Summary
We are seeking a compassionate and highly skilled Registered Nurse (RN) Case Manager to join our dedicated hospice team in the beautiful Baker City this role you will be the primary advocate for patients and their families coordinating comprehensive end-of-life care that honors the patients dignity and comfort. You will manage a dedicated caseload providing clinical assessments symptom management and emotional support in the patients home or residential facility.
Key Responsibilities
- Case Management: Act as the primary coordinator for the interdisciplinary team (Social Workers Chaplains Aides and Physicians) to ensure a holistic approach to patient care.
- Clinical Assessment: Perform initial and ongoing nursing assessments to evaluate physical emotional and spiritual needs.
- Symptom Management: Administer medications and clinical treatments focused on pain control and comfort adhering to physician-ordered care plans.
- Education & Support: Empower families and caregivers by teaching proper care techniques explaining the dying process and providing resources for grief and bereavement.
- Documentation: Maintain accurate real-time electronic medical records (EMR) to ensure compliance with state and federal hospice regulations.
- Supervision: Provide clinical oversight and guidance to Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs) within the care team.
Requirements & Qualifications
- Licensure: Current unencumbered Registered Nurse (RN) License in the state of Oregon.
- Education: Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
- Experience: * Minimum of 12 years of clinical nursing experience.
- Prior experience in Hospice Palliative Care or Home Health is strongly preferred.
- Certifications: Valid CPR/BLS certification. CHPN (Certified Hospice and Palliative Nurse) is a plus.
- Travel: Must have a valid drivers license reliable transportation and active auto insurance for travel within Baker County and surrounding areas.
- Skills: * Exceptional clinical judgment and critical thinking.
- Strong interpersonal communication and the ability to handle emotionally charged situations with grace.
- Proficiency with EMR software (experience with Homecare Homebase is a bonus).
Physical Requirements
- Ability to lift/move up to 50 lbs and perform physical tasks (bending kneeling standing) required for patient care.
- Comfortable working in various home environments and traveling in all weather conditions typical of Eastern Oregon.
