Job Summary

Job Description:

We are currently seeking a Client Relation Specialist to join our team at AmeriHealth Hospice. As a Client Relation Specialist you will play a crucial role in providing support to patients and their families during the end-of-life phase. Your primary responsibility will be to ensure that the emotional physical and spiritual needs of the patient and their families are met with compassion and professionalism.

Job Responsibilities:

Build and maintain strong relationships with clients patients and their families to provide continuous support and guidance.
Act as a liaison between the hospice team and the clients to coordinate care and services effectively.
Assess the unique needs of each patient and family unit and develop personalized care plans.
Provide emotional support and counseling to clients and families throughout the end-of-life process.
Ensure all client information and documentation are accurately maintained and up to date.
Collaborate with healthcare professionals and volunteers to deliver comprehensive care to patients.
Participate in interdisciplinary team meetings to discuss patient progress and address any concerns.
Maintain a high level of confidentiality and professionalism in all interactions with clients and their families.

Essential Qualifications:

Bachelors degree in psychology social work nursing or a related field.
Proven experience in a client-facing role preferably in a healthcare or hospice setting.
Excellent communication and interpersonal skills.
Empathetic and compassionate approach to supporting patients and families in difficult situations.
Strong organizational skills and attention to detail.

Desired Experience:

Experience working in end-of-life care or hospice services.
Familiarity with medical terminology and healthcare procedures.
Previous experience providing emotional support and counseling to individuals and families.
