- $70000 to $75000 year (based on experience)
Your Schedule!
- Monday through Friday from 8:30 am to 5 pm plus some weekends as needed
- Local Travel will be required to client homes and facilities mileage reimbursement provided

- You have at least1 year of paid RN Nurse experience (3 5 years of clinical experience with 1 year of experience in a home health agency primary care clinic or health facility preferred.)
- You have a current unrestricted Registered Nursing (RN) license in the state of Illinois
- You have a valid vehicle with insurance & can reliably commute to this jobs location
- You understand regulatory compliance with state and federal regulations and can adhere to HIPAA and maintain client confidentiality

Your Responsibilities!
- Conduct Nursing and/or Needs Assessment at the start of care
- Conduct meet and greets for new field staff placements
- Monitor and coach home nursing and home services field staff
- Review nursing notes and coach field staff to improve quality
- Review weekly care notes for quality of documentation and changes in patient condition requiring revisions to the Plan of Care.
- Nurse Case management: contact families coordinate care field visits home and facility communicate changes in Plan of Care. Obtain the physicians signature approval as necessary.
- Foster trusted relationships with peers and referral sources in facilities
- Rotating on-call schedule
- Administer prescribed medications and treatments per approved nursing techniques
- Conduct field supervisory visits and perform competency reviews on field staff skills as needed
- Perform field injections medication set-ups and blood draws as time permits
- Observe patient record significant conditions and reactions and notify supervisor or physician of patients condition and reaction to drugs treatments and significant incidents
- Take and record patients vital signs to detect deviations from normal and assess the condition of the patient
- Respond to life-saving situations based on nursing standards policies procedures and protocol
- Document nursing history and physical assessment for assigned patients
- Initiate a patient education plan according to the individualized needs of the patient as prescribed by the physician and/or Agency policyincluding patient and family instruction
- Work with the Physician and family to ensure that Discharge planning is communicated to the patient
- Other duties as assigned
