Overview
The Targeted Case Manager provides Rule 79 Case Management services to eligible persons primarily referred directly from Hennepin County or self-referral who have been diagnosed with a serious and persistent mental illness. TCM services assist with access to medical social and other services. Targeted Case Managers ensure continuity of care coordination and monitoring of services discharge planning housing support and intensive goal planning through the development of the Individual Community Support Plan. The Targeted Case Manager would utilize a person-centered approach to work in partnership with persons served with the goal of improved functioning through acquisition of necessary supports skill development and continuity of services.

Responsibilities
Along with using a person-centered approach and building meaningful relationships our Targeted Case Managers will have the following job duties:
- Conducting a variety of clinical assessments to determine needed areas of the ICSP (Individual Community Support Plan)
- Development of the Individual Community Support Plan (Treatment/Rehabilitation Plan)
- Referral and linkage to community resources
- Monitoring and coordinating of resources supports and services
- Civil Commitment responsibilities
**building meaningful relationship; knowledge and skill with benefit acquisition and management; resource and referral; knowledge of Rule 79 clinical documentation.
Qualifications
- Holds a bachelors degree in one of the behavioral sciences or related fields from an accredited college or university and/or has at least 2000 hours of supervised experience in the delivery of services to persons with serious and persistent mental illness; or holds a graduate degree in one of the behavioral sciences or related fields.
- Individuals with 2000 hours of experience in the delivery of mental health services to adults All staff are supported in gaining certification as a Certified Psychiatric Rehabilitation Practitioner.
- The agency encourages staff to seek license at the highest standard permitted by their education and experience.
