About the role
The practice lead - clinical provides expert clinical leadership across our community-based care services supporting our teams to deliver safe connected person-centred care. This role champions implementation of clinical and care governance frameworks provides clinical expertise in the design and coordination of services identification and management of recipient health-care related risks and support workforce capability in developing educational resources to our workforce.
Key responsibilities include but are not limited to:
- leading the development implementation and continuous review of clinical governance frameworks policies procedures and care models aligned with best practice
- providing expert guidance to ensure services are delivered in accordance with safe highquality care standards with a specific focus on child and family youth disability and mental health services
- leading implementing and monitoring quality improvement initiatives that strengthen care delivery promote safety improve recipient outcomes and enhance organisational performance related to safe care practices
- driving a proactive approach to compliance and risk management including conducting internal audits supporting the management of escalated concerns and coordinating preparation for regulatory assessments and accreditation activities
- building and maintaining effective partnerships with internal teams and external stakeholders to support integrated service delivery and improve outcomes for recipients
- analysing and interpreting data from incidents audits and quality reports to identify trends risks and opportunities using insights to inform continuous improvement strategies
- providing education coaching and specialist support in clinicalrelated practices across community services promoting safe care delivery safe administration and recipient wellbeing
- delivering clinical leadership and oversight of recipientrelated risk in community settings ensuring early identification assessment escalation and management of changes in health status to support safe highquality riskinformed care
About you
To be considered for this rewarding role you will have:
- clinical expertise and sound judgement with a commitment to evidence-based practice
- experience working in community-based care and/or demonstrated knowledge of the Human Service Quality Framework (HSQF) and National Disability Insurance Scheme (NDIS) Practice Standards
- demonstrated experience providing clinical guidance and mentoring to non-clinical workforces to promote safe practice clear role boundaries and timely escalation of care concerns
- highly developed personcentred communication skills
- strong capacity to engage influence and collaborate effectively across multiple stakeholders
- proven capability to think critically and approach challenges with innovation
- a track record of driving and sustaining continuous improvement in clinical practice
- demonstrated experience in cultivating a safe supportive and wellbeing-focusedenvironment including promoting safe practice supporting psychological safety and proactively managing risks
Role requirements
- bachelor of nursing and AHPRA registration to practice as a nurse in Australia
- experience in the community services industry (highly desirable)
- travel to selectability sites may be required from time to time
- current nationally recognised first aid and CPR certificates (or ability to obtain within the first month of employment)
- a Child Safety & Personal History Screening check
- a Positive Notice Working with Children (blue card) (or the ability to obtain)
- NDIS Worker Screening Clearance (or the ability to obtain)
- NDIS Worker Orientation Module Quality Safety & You completion certificate (or the ability to obtain)
- a current minimum P1 C class Queensland drivers licence
- roadworthy vehicle with comprehensive insurance
