Position Overview:

The Case Manager provides case management to older adults providing assistance in care coordination patient support in maintaining income housing basic needs and independent living skills for NARA NWs older adult population. Works as a part of the mental health team and in close collaboration with the addictions outreach team medical services and other NARA NW programs. Assists clients and therapists in developing strength-based service plans for case management and community-based services

What you will do:

  • Help elders navigate the care delivery system including systems of health and behavioral health housing insurance and transportation.
  • Monitor food and housing security
  • Meet with clients in hospital stays when appropriate for continuation of care.
  • Provide case management and community services to elders living with mental health and/or substance use disorders that significantly affect their functioning in the community setting
  • Assist with obtaining benefits such as insurance and Social Security benefits job training housing health care socialization and other community resources as identified in a strengths-based plan
  • Provide community-based skills training such as finance management medication management disease management self-advocacy use of public transportation nutrition hygiene etc.
  • Refer to appropriate mental health or addiction services that address elders needs
  • Assist with connection to cultural resources and events including transportation and support at events as desired.
  • Assist clients in learning about and accessing cultural resources and activities
  • Provide a warm hand off for clients who are referred to resources e.g. attending first appointments with Elders addictions intake housing cultural activities etc.


Qualifications

We would like to hear from people that have:

  • Either a High School Diploma or General Educational Development (GED) certificate required.
  • Credentialed as a QMHA through MHACBO required.
  • Bachelors degree preferred.
  • Previous experience with older adults and/or Native American/Alaska Native populations strongly preferred.
  • Ability to be credentialed as a peer support provider or recovery mentor would be considered an asset.
  • Desire to serve elders in a supportive capacity previous experience preferred.
  • Additional training around senior services culture and knowledge of community resources and entitlement programs preferred.
  • Knowledge and understanding of mental illness medications symptoms and treatments.
  • Understanding of common health and behavioral health issues for older patients.
  • Ability to engage and interact positively with clients to promote strengths and improve health and wellbeing.
  • Initiative to communicate effectively in coordinating care.
  • Excellent communication skills.
  • Knowledge and skill in working in a multi-cultural environment.
  • Good understanding of professional roles and boundaries.
  • Must pass a pre-employment or for cause drug tests.
  • Must pass criminal background and DMV checks.
