Job Summary
RN Transition of Care Specialist Emergency Services
Location: Bronx NY 10461
Assignment: 13 Weeks
Schedule: Day Shift 40 Hours/Week
Department: Emergency Department
Weekly Gross: $2779.87
Position Summary
This role supports patient transitions and throughput by providing care coordination utilization management and discharge planning within the Emergency Department.
Key Duties
- Facilitate patient transitions from the ED
- Perform utilization reviews and care coordination
- Identify discharge barriers and arrange services
- Collaborate with interdisciplinary teams
- Ensure documentation accuracy and compliance
Qualifications
- NY RN License
- 5 years RN experience
- Care coordination and discharge planning background
- ASN/ADN required
- BLS required
Preferred
- BSN/MSN
- Case Management experience
