Patient Care Coordinator, Specialty Evernorth

Must reside in Pennsylvania Ohio and West Virginia to be consider for the position.

OnSite Training:
This role requires 6-8 weeks of onsite training at the following location:
1001 Oakdale Road Oakdale PA 15071

Responsible for supporting the clinical management and refill coordination of specialty pharmacy patients under the supervision of a licensed pharmacist. This role involves direct communication with patients caregivers and healthcare professionals to ensure accurate and timely delivery of medications.

Responsibilities

• Coordinate patient deliveries by making outbound and receiving inbound calls from patients caregivers and medical staff.
• Conduct follow-up calls as directed by a licensed pharmacist.
• Assist the specialty pharmacy team with tasks such as:
  • Evaluating product usage
  • Managing and documenting supply inventory
  • Setting up medication orders
  • Performing verbal patient assessments
• Maintain and update patient records medication profiles and complete assessments under pharmacist supervision.
• Accurately document all interactions in patient medical records.
• Meet or exceed quality and productivity standards (e.g. 100125 form setups daily).
• Deliver exceptional customer service and maintain a professional service-oriented approach in all interactions.
• Act as a liaison between specialty pharmacy operations and patients caregivers or medical professionals.
• Adhere to system-wide competencies and behaviors.
• Perform additional duties as assigned.

Requirements

• High school diploma or GED required.
• Pharmacy Technician Certification preferred.
• Prior experience in specialty healthcare preferred.
• Proficiency in Microsoft Office Suite.

our Pharmacy and Care Delivery organization. Our team focuses on
specialty pharmacy and infusion services in partnership with hospitals and
health systems.

This role is also anticipated to be eligible to participate in an annual bonus plan.
