Your Role: Home Health Physical Therapist
As a Physical Therapist in a home health setting youll work one-on-one with patients to restore mobility reduce pain and prevent falls empowering them to live safely and independently at home.
Key Responsibilities:
Conduct in-home PT evaluations and follow-up visits
Design and implement individualized rehabilitation and treatment plans
Improve patients strength balance mobility and functional independence
Educate patients and caregivers on safe movement and equipment use
Document visits using WellSky Kinnser EMR
Coordinate care with physicians RNs case managers and the interdisciplinary team
What Were Looking For:
California-licensed Physical Therapist (PT)
CPR certification (AHA-approved)
At least 1 year of clinical experience (Home Health experience preferred)
Organized compassionate and reliable
Valid drivers license auto insurance and reliable transportation
Strong communication skills and clinical decision-making
