JOB SUMMARY

Provides County-wide information assistance and access to agency and community resources for older adults. Assesses plans implements coordinates follows up and evaluates options and services to meet clients needs.

MINIMUM QUALIFICATIONS

Bachelors degree from an accredited college or university in a Human Services field or other closely related field; OR an equivalent combination of related education and experience.

A valid Drivers License accepted by the State of Utah is required at the time of hire. A valid Drivers License issued by the State of Utah must be obtained within six (6) months of hire to operate a County vehicle.

Due to the nature of this position the successful applicant must pass a required background check through fingerprinting and enrollment in the continuous RAP Back (Record of Arrest and Prosecution Back) program in accordance with current County Human Resources policy requirements.

ESSENTIAL FUNCTIONS

  • Provides information and support to clients and families/friends seeking access to services. Consults with clients and families to determine eligibility requirements. Advocates for clients linking them to services and referring to internal and external resources.
  • Responds daily to telephone on-site visits walk-in inquiries and concerns relating to health and well-being including emergency or potentially life-threatening
  • Provides comprehensive assessments in overall health environment and formal and informal support status and annual reassessments for older adults seeking or participating in division services eligibility for homebound older adults seeking Meals on Wheels or other
  • Facilitates support and follow-up between clients and staff as needed.
  • Develops and maintains working relationships with community partners who provide services to older adults.
  • Coordinates and communicates with Adult Protective Services Law Enforcement Fire Departments Mobile Crisis Outreach Teams and other agencies to ensure the safety of older adults as needed.

  • Aging process chronic illness and related conditions
  • Community resources agencies health care delivery systems and rules and regulations
  • Interviewing methods and techniques
  • Computer and office software related to job-specific duties

  • Organize and schedule work efficiently
  • Provide professional customer service to the community
  • Demonstrate critical thinking
  • Prioritize and perform multiple tasks simultaneously
  • Communicate effectively both verbally and in writing
  • Utilize web-based information for researching
  • Accurately assess the social mental and emotional status of older adults
  • Resolve problems successfully
