Where youll be working
Manning Base Hospital is seeking a dedicated Social Worker to deliver high-quality clinical services and professional leadership within a multidisciplinary team. Based in the Lower Mid North Coast Sector of Hunter New England Local Health District this role has primary responsibility for Social Work services within Renal Services with opportunities to contribute across other areas of the hospital as required.
Youll be joining a supportive and collaborative environment focused on delivering compassionate patient-centred care to individuals and their families.
About the role
In this role you will work closely with patients and their families/carers to support wellbeing and quality of life throughout the course of chronic kidney disease. You will provide psychosocial assessment counselling and practical support assisting individuals to manage the impacts of illness maintain independence pursue personal goals and adapt to ongoing treatment and lifestyle changes.
Youll also support families and carers through periods of adjustment contributing to functional rehabilitation and continuity of care across the patient journey.
Comprehensive orientation and onboarding will be provided along with ongoing support from the broader team and access to continued education and professional development opportunities.
About you
You are a skilled and compassionate Social Worker with strong clinical knowledge of renal disease and its psychosocial impacts. You bring the ability to assess complex needs develop individualised care plans and deliver counselling and emotional support to patients and their families.
You are confident working in complex and emotionally demanding situations advocating for patients and navigating community financial and government support systems. You demonstrate a commitment to ethical practice accurate documentation and confidentiality.
You work effectively within a multidisciplinary team and bring excellent communication problem-solving and adaptability skills. A respectful culturally responsive approach is essential with the ability to work sensitively with diverse communities including Aboriginal and Torres Strait Islander peoples.
Requirements:
Be rewarded with up to $10000 in the first 12 months PLUS up to $10000 each year thereafter in fortnightly payments under the Rural Health Workforce Incentive Scheme.*
