RN Case Manager
Salary: $79000$83000 annually
Full-Time No Weekends Limited On-Call
Spoon River Home Health is looking for a compassionate and reliableRegistered Nurse (RN)to join our team. This full-time position involves working closely with supervisors and the interdisciplinary care team to plan implement and evaluate high-quality person-centered nursing care.

Responsibilities:
Deliver and coordinate nursing care based on individualized plans
Collaborate with the care team to ensure quality outcomes
Document care clearly and accurately
Support and educate patients and families
Maintain compliance with clinical protocols and safety standards
Qualifications:
Current RN license (Illinois)
Minimum of 1 year nursing experience
Strong time management and organizational skills
Dependable and self-motivated
Committed to delivering compassionate person-centered care
Home health experience preferred but not required
