Your Role (Varies by Discipline):

Provide skilled compassionate care to patients in their homes
Collaborate with an interdisciplinary care team to support patient goals
Complete accurate and timely documentation
Educate patients and families to promote safety independence and confidence
For On-Call RNs: Provide clinical support and guidance during assigned on-call hours

What You Need to Succeed:

Active Pennsylvania License (RN LPN PT OT SLP CNA as applicable)
BLS Certification (for licensed clinical roles)
Valid Drivers License & Reliable Transportation
Strong Communication & Documentation Skills
Ability to Work Independently
Home Health Experience (Preferred Not Required)
