Shift: Full-time Monday through Friday 8:00 AM to 5:00 PM
Job Overview:
We are seeking a Licensed Master Social Worker (LMSW) to join its Home Health and Hospice team serving both Kansas and Missouri locations. This is a full-time flexible role focused on providing hospice and home health social work services to adult and geriatric patients in their homes. The LMSW plays a critical role in supporting patients and families through psychosocial assessments care planning education and emotional support while collaborating closely with interdisciplinary care teams. This position offers long-term stability with a well-established agency and a strong emphasis on holistic patient-centered care.
Coverage Details:
- Coverage Area: North Kansas City including Liberty Platte City Excelsior Springs Blue Springs and Independence
- Average Daily Mileage: Approximately 50 to 100 miles per day typically 5070 miles
- Mileage Reimbursement: $0.45 per mile excluding mileage to and from home
Key Responsibilities:
- Provide hospice and home health social work services through in-home patient visits
- Conduct psychosocial assessments and develop care plans tailored to patient and family needs
- Support patients and families with counseling education and resource coordination
- Collaborate with interdisciplinary team members to ensure comprehensive care delivery
- Assist patients and families with advance care planning and end-of-life decision-making
- Document assessments interventions and care plans using electronic charting systems
- Monitor patient needs and communicate changes to the care team
- Promote holistic non-pharmacologic approaches when appropriate addressing nutrition lifestyle and psychosocial factors
- Maintain compliance with all federal state and organizational regulations
Required Qualifications:
- Master of Social Work (MSW) degree from an accredited program
- Active LMSW license in Missouri and/or Kansas as applicable
- Minimum of 1 year of experience working in a healthcare setting hospice experience preferred
- Current CPR certification
- Valid drivers license and active auto insurance
- U.S. citizenship or valid green card
- Strong communication assessment and time management skills
- Ability to work independently in a home-based care environment
