Job Summary

Job Summary (List Format):

- Restore function and prevent/minimize disability following disease or injury.
- Help clients achieve maximum performance/function within their capabilities.
- Evaluate and treat patients using medically prescribed physical therapy programs.
- Supervise Physical Therapy Assistants as needed.
- Make initial admission and evaluation visits.
- Notify Clinical Team Manager of OASIS scores within established timelines.
- Document observations clinical findings problems interventions/treatments goals and discharge plans.
- Perform additional duties as assigned consistent with skills training and organizational mission.
- Provide services within assigned territory: Milpitas Saratoga Monte Sereno Los Gatos Campbell San Jose Morgan Hill San Martin Gilroy Coyote Point Sunnyvale and neighboring cities.
- Work primarily weekdays (8am-5:30pm) with schedule flexibility based on patient availability.
- Work one weekend day per month as required.
- Specialty: Rehabilitation Therapy Physical Therapist.
- Certification required: BLS/BCLS.
- Minimum requirement: One year of Home Health experience.
