Job Title: Care Manager (RN)
Employment Type: Contract (6 months)
Extension/Conversion: Yes potential to extend and/or convert
Work Location: Remote Ohio only (West Central: Montgomery Greene Clark)
Field-based: 50 60% travel for member visits
Schedule
- Training: Monday Friday 8:00 AM 5:00 PM EST
- 4 weeks (virtual classroom & 1:1 training)
- Cameras required no time off permitted during training
- Regular Schedule: Monday Friday 8:00 AM 5:00 PM EST
Key Responsibilities
- Conduct 5 7 member visits per week (home facility or community-based)
- Develop and manage individualized care plans
- Coordinate care across providers services and resources
- Document all visits and activities within 24 hours
- Review daily tasks emails voicemails and calendars starting at 8:00 AM
- Meet turnaround times for care management processes
- Complete all required trainings on time
- Collaborate with interdisciplinary teams to support member independence
Performance Metrics
- 5 7 completed member visits weekly
- Documentation completed within 24 hours
- Timely completion of processes and trainings
Education
Required:
- Degree from an accredited School of Nursing OR Bachelors Degree in Nursing with 2 4 years of related experience
- Preferred: 2 4 years of related experience
Licensure
- Required: Active Ohio RN License
