Job Summary
Join Interim HealthCare (Home Health)- Maine as an RN Case Manager in Portland ME where you will play a vital role in coordinating and managing patient care to ensure optimal health outcomes. Responsibilities
- Assess patient needs and develop individualized care plans.
- Coordinate with healthcare providers patients and families to facilitate quality care.
- Monitor patient progress and adjust care plans as necessary.
- Provide education and support to patients and their families regarding care management.
- Document all case management activities accurately and timely in compliance with organizational policies.
- Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
Requirements
- Current and valid RN license in the state of Maine.
- Experience in case management or related nursing role preferred.
- Strong assessment organizational and communication skills.
- Ability to work independently and collaboratively in a fast-paced environment.
- Proficiency with electronic health records and case management software is a plus.
