Our client is seeking an Adult Social Worker to join their Young People Transitions to Adulthood Team based in Oldham.
What will your responsibilities be
- Support young people preparing for adulthood completing holistic assessments that identify aspirations needs risks and the support required as they move from childrens to adult services.
- Coordinate multiagency transition plans working closely with education health adult social care and community providers to ensure continuity of care and smooth handover.
- Promote independence and personcentred outcomes developing plans that build life skills enhance wellbeing and empower young people to make informed decisions about their future.
