This position is eligible for the Education Debt Reduction Program (EDRP) a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval award amount (up to $200000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
Delivers fundamental knowledge-based care to assigned clients at a Community Living Center. Assesses plans implements and evaluates restorative nursing care. Administers medications and procedures while coordinating care focused on patient education and collaboration with the interdisciplinary team.
Completes Restorative Nursing assessments and develops individualized care plans
Completes Restorative Nursing sections of the Minimum Data Set (MDS) documentation
Administers medications and treatments prescribed by licensed healthcare providers
Provides health teaching counseling and care supportive to patient well-being and illness prevention
Conducts chart audits and compiles data for Performance Improvement (PI) indicators
Provides training and education to disciplines regarding the Restorative Nursing Program
Develops action plans to correct identified deficiencies in care delivery
Educates older adults and families about chronic illness and mental health treatment options
Assists in coordinating care among caregivers and interdisciplinary team members
Participates in quality care improvement activities and Unit Based Council initiatives
