Candidates can live anywhere in or near the state of (California).

Must be able to travel and provide case management services primarily in California.

JOB PURPOSE:The Home Study and Post-Release Services (HSPRS) Case ManagerLevel 2willprovidecase management and supportive services to unaccompanied immigrant children released from federal HSPRS Case Manager will work in collaboration with program staff and supervisors to cultivate a client-centered and trauma-informed approach to service provisions.

ESSENTIALJOBRESPONSIBILITIES:

• Conduct home visits and phonecontacts with the dual purpose of assessing safety and appropriatenessofchildsplacementwiththeircaregiver(sponsor)andconnectingchild to services in the community to address risk factors.
• Ongoingcasemanagementwithafocusonscreeningandmakingappropriatereferralsto address childs needs in the following domains: Placement issues safety issues education health mental health legal/immigration proceedings substance abuse independent living criminal/juvenile justice child enrichment activities.
• Conductage-appropriatecomprehensiveassessmentinachildcenteredtrauma-informedmannerand documented in an online case management system.
• Providecrisisinterventionservicesdirectlyasneededorconnectfamilytoservicestoensureminorssafety andwell-beinginacrisissituation.
• Activelyestablishenhanceandmaintainliaisonrelationshipswithkeygovernment agencies and non-profits/ community organizations.
• Complywithpoliciesandproceduresgoverningtheprogramandensurethattheprogram isoperatingat maximum efficiency.
• Meetorexceedprogramproductivityexpectationson aregularandsustainedbasis.
• Otherdutiesasassigned.
• DemonstrateExceptionalcustomerserviceinEverythingyou dobyplacingthechildfamilyVeteranorclientfirsttosupportourmissiontoEmpowerpeopletobuildbetterlivesforthemselvestheir families and their communities.

ESSENTIALQUALIFICATIONS:

• Minimumof1yearsofexperiencerelevanttothepositionandworkingwithvulnerableormigrant populations; 3 years preferred.
• ProficiencyinSpanish/English required (writtenandspokenlanguageskills).
• Excellentverbalandwrittencommunicationskills.
• Abilitytohandleconfidentialmaterialin aresponsiblemanner.
• Ableandwillingtotraveldomesticallyupto 50%oftime.
• ProficiencyinMicrosoftOfficeapplications.

EDUCATION:Bachelorsdegreeintheeducationpsychologysociologyorotherbehavioralsciencesequivalent.

EXPERIENCE:SeeEssentialQualificationsandEducation.

ATTENDANCE:Mustmaintainregularandacceptableattendanceatsuchlevelasisdeterminedintheemployerssolediscretion.

LICENSES:DriversLicense withclear record.

VEHICLE:Must have daily use of a vehicle without prior notice.

OTHER:Must be available and willing to travel to various locations and with such frequency as the businessneeddictates.
