Responsible/accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses plans implement and evaluate care based on age-specific components. Assume responsibility for the coordination of care focus on patient education self management and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.
Responsible for providing competent evidence base care to assign patients. Provides patient management care coordination and disposition planning for patients. Assist veterans family members and caregivers with receiving the most appropriate options and services to meet their complex healthcare needs. Coordinate care with multiple providers across all levels and sites of care. Coordination also includes assuring services interface within the veterans affairs and community programs to provide a continuity of care to patients.
Manages potential Lung Nodule Registry (LNR) enrollment by performing chart reviews to verify eligibility and appropriateness. Organizes and lead and services to educate and share information about the Lung Nodule Registry program with clinical staff. Facilitates follow up referrals to evidence based smoking sensation care. Optimize patient centered care practices through care management screening exams procedures and clinical appointments. Maintains accountability for following imaging studies and their recommendations. Collects and analyzes program data from a variety of sources for meetings and presentations. Court dates and schedules LNR primary care/nursing visits for education at dissemination of education over materials. Assist with planning LNR promotional events. Provides timely communication and reports as needed to the interdisciplinary team. Performs initial and ongoing monitoring and tracking of abnormal images/lung nodules and communicating with providers to ensure that they are alerted to abnormal findings when necessary. Responsible for other duty supporting LNR assigned.
Addresses psychosocial as well as nursing and medical needs of patience and their family/caregivers through participation with the interdisciplinary carotene. Coordinate an expedite care from diagnosis to treatment. Evaluate care and outcomes to ensure timely inappropriate provision of services. Advocate for holistic care through patient/family education and provider interaction and implements and educational plan to meet a patient care needs. Collects analyzes and reports program evaluation for assessment on the impact of implementing long nodule registry and to make evidence based improvements.
Demonstrates leadership and delivering and improving holistic care through collaborative strategies with others. Evaluate practice in an ongoing process based on evidence. Provides peers with an informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional nursing perspective and discussions with the interdisciplinary team. Partners with others to affect change and produce optimal outcomes. Supports colleagues through knowledge sharing and provide safe quality nursing care. Shares educational findings experiences and ideas with peers. Questions clinical practice for the purpose of providing evidence based care. Participates in activities and strategies to sustain an evidence based practice culture.
