We are seeking a full-time Speech Therapist to join a collaborative and supportive rehabilitation team within a large Skilled Nursing Facility. The therapist will work closely with Speech Therapists Physical Therapists Occupational Therapists and nursing staff to deliver high-quality patient-centered care.
The facility includes multiple skilled nursing units a sub-acute unit and a secured memory care unit offering a diverse and engaging clinical environment.
Key Responsibilities-
Provide speech-language pathology services to residents in a skilled nursing and memory care setting

Evaluate treat and document patient progress according to established care plans
-
Collaborate with interdisciplinary therapy and nursing teams
-
Participate in care planning and team meetings as appropriate
-
Maintain accurate and timely documentation using EMR systems
-
Day shift
-
Approximately 8:00 AM 4:30 PM (schedule may vary)

Active state Speech Therapy license
-
Experience in Skilled Nursing Facility setting preferred (new graduates encouraged to apply)

Willingness to learn and adapt in a team-oriented environment

Strong communication and interpersonal skills

Experience with NetHealth PCC or PointClick preferred
-
Training and mentorship available for therapists new to SNF or these EMR systems
-
Current BLS/BCLS required

Supportive experienced therapy team

Mentorship available for therapists new to SNF settings

Stable full-time schedule in a collaborative work environment
