Speech-Language Pathologist (SLP) Skilled Nursing Facility Contract Quick Start
We are seeking a qualified Speech-Language Pathologist (SLP) to join a Skilled Nursing Facility on a contract basis. This role is ideal for an SLP with prior SNF experience who is confident working in a fast-paced rehabilitation environment.
Position Overview The Speech-Language Pathologist will evaluate diagnose and treat residents with communication cognitive and swallowing disorders collaborating closely with the interdisciplinary rehab team to support patient-centered care.
Responsibilities -
Evaluate and treat speech language cognitive and swallowing disorders

Develop and implement individualized treatment plans

Document evaluations progress notes and discharge summaries per facility standards

Collaborate with PTs OTs nursing staff and physicians

Ensure compliance with all clinical regulatory and documentation requirements
Requirements -
Active California Speech-Language Pathologist license

Minimum 1 year of Skilled Nursing Facility (SNF) experience

Experience with Rehab Optima / Net Health preferred

Strong clinical judgment documentation and time-management skills

Ability to work independently and as part of a multidisciplinary team
