Job Summary

Speech-Language Pathologist (SLP) Skilled Nursing Facility

Contract Quick Start

We are seeking a qualified Speech-Language Pathologist (SLP) to join a Skilled Nursing Facility on a contract basis. This role is ideal for an SLP with prior SNF experience who is confident working in a fast-paced rehabilitation environment.

Position Overview

The Speech-Language Pathologist will evaluate diagnose and treat residents with communication cognitive and swallowing disorders collaborating closely with the interdisciplinary rehab team to support patient-centered care.

Responsibilities
  • Evaluate and treat speech language cognitive and swallowing disorders

  • Develop and implement individualized treatment plans

  • Document evaluations progress notes and discharge summaries per facility standards

  • Collaborate with PTs OTs nursing staff and physicians

  • Ensure compliance with all clinical regulatory and documentation requirements

Requirements
  • Active California Speech-Language Pathologist license

  • Minimum 1 year of Skilled Nursing Facility (SNF) experience

  • Experience with Rehab Optima / Net Health preferred

  • Strong clinical judgment documentation and time-management skills

  • Ability to work independently and as part of a multidisciplinary team
