Job Overview

We are seeking a licensed Physical Therapist (PT) to join a leading home health provider delivering high-quality one-on-one care to patients in their homes across Salem McMinnville and Eugene OR. This role focuses on improving patient mobility independence and overall quality of life through personalized treatment plans and collaborative care.

Key Responsibilities

Conduct comprehensive in-home patient evaluations and develop individualized treatment plans
Provide therapeutic exercises manual therapy and functional mobility training
Educate patients and caregivers on safety home exercise programs and long-term care strategies
Collaborate with an interdisciplinary team including nurses occupational therapists social workers and physicians
Accurately document patient progress and treatment plans in EMR systems
Ensure compliance with HIPAA safety and clinical standards
Travel between patient homes; mileage reimbursement provided

Qualifications

Active Oregon Physical Therapist license (Masters or Doctorate preferred)
Open to new graduates; home health experience is a plus
Current CPR/BLS certification
Valid drivers license with insured reliable transportation
Strong clinical communication and patient education skills
Ability to work independently and manage a dynamic caseload
