The Quality Nurse Coordinator is a Registered Nurse responsible for supporting clinical quality initiatives transitional care management and care coordination to improve patient outcomes and ensure compliance with regulatory and payer quality standards. This role emphasizes post-discharge outreach care gap closure quality measure support and clinical task management while upholding the organizations Qssentials Standards of Excellence. The Quality Nurse Coordinator collaborates closely with providers clinic staff and external partners to deliver timely patient-centered and compliant care across all age groups.
Provide timely telephonic outreach to patients discharged from hospitals or skilled nursing facilities.
Assess post-discharge needs including medication reconciliation and symptom review.
Schedule and coordinate TCM visits with the patients Primary Care Provider (PCP).
Document all outreach and care coordination activities in the electronic medical record (EMR).
Support quality measures primarily for PCPs with potential expansion to specialty services (Cardiology Pulmonology Gastroenterology).
Collaborate with providers clinic staff and insurance companies to improve quality metrics and close care gaps related to preventive services chronic disease management and wellness requirements.
Navigate payer portals for quality reporting audits data submission and documentation.
Assist with audits reporting deadlines and performance improvement initiatives.
Triage incoming patient calls and portal messages; identify urgent concerns and escalate appropriately.
Provide patient education related to test results preventive care and follow-up instructions.
Assist providers by managing clinical task queues and ensuring timely patient communication.
Manage clinical task buckets including:
Medication refill requests per protocol
Patient portal messages
Diagnostic and lab result follow-up
Ensure accurate and timely documentation of all patient interactions in the EMR.
Support vaccine program compliance (e.g. Vaccines for Children VFC).
Monitor vaccine storage requirements and maintain temperature logs.
Coordinate thermometer calibration and maintain required documentation.

Current licensure as a Registered Nurse required
BSN or higher degree preferred
35 years of experience in clinical or community health settings preferred
Outpatient or ambulatory care experience preferred
Experience with quality measures audits or insurance reporting strongly preferred
Proficiency with EMR systems and payer portals
Current Illinois Registered Nurse license
Basic Life Support (BLS/CPR) for Healthcare Providers
Travel to other Vista Health sites may be required
