JOB SUMMARY
The HC Supervisor will directly supervise Home Coordinators and Leads as well as indirectly supervise Direct Support Professionals. This individual is the primary source of communication and information for keeping the Regional Director informed of all problems or changes in our homes. This individual will also serve in and on-call rotation. You will also be expected to maintain accurate records as required by Buckeye Residential Solutions and any other governing agency such as DODD ODHS and County Case Management.
PRIMARY RESPONSIBILITIES
Training:
- Responsible for interviewing hiring evaluations and maintaining proper training and development of high-quality staff for Home Coordinators and Leads.
- Ensure direct reports are receiving the required annual trainings.
Supervision:
- Directly Supervise all Mahoning and Trumbull County direct Home Coordinators and Leads.
- Conduct employee evaluations for direct reports.
- Overall responsibility for scheduling coordination and compliance in accordance with company policies.
- Coordinate and facilitate communication to members of the clients team which may include service and support administrators (SSAs) day service representatives and family or legal guardians.
- Interact directly with clients their guardians behavioral specialists and SSAs as needed.
- Supervise the completion of Clock-Ins HPC sheets BSP Data Tracking Sheets Unusual Incident Reports and Major Unusual Incident Reports.
- Ensure that all Home Coordinators/Leads have adequate home training to ensure that all DSPs are being properly trained in the home.
- Trained in Primary Solutions
- Ensure that daily medications are being passed by nursing or certified staff.
- Track trends and patterns from documentation and incident reports.
- Perform house/ vehicle checks.
Administration:
- Report all UIs and MUIs as required by law.
- Weekly review and where necessary correction of alldocuments and other information required for regulatory compliance purposes to ensure adequate documentation.
- Lead attend and participate in scheduled staff meetings designated training and in-service sessions.
- Coordinate weekly or as needed meetings with Home Coordinators/Leads.
- Service and Support data entry
- Perform central scheduling.
- Keeps equipment operating by establishing and enforcing operating and preventive maintenance rules; arranging for repairs.
Medical/ Physical:
- Adhere to confidentiality guidelines as outlined by organization policy and HIPPA regulations.
- Coordinate and facilitate communication to all members of the team which may include service and support administrators (SSAs) day service representatives and family or legal guardians.
- Interact directly with clients their guardians behavioral specialists and SSAs.
- Track trends and patterns from documentation and incident reports.
- Conduct employee evaluations for direct reports.
- Report all UIs and MUIs as required bylaw.
- Weekly review and where necessary correction of all documents and other information required for regulatory compliance purposes to ensure adequate documentation.
- Adhere to confidentiality guidelines as outlined by organization policy and HIPPA regulations.
Other Duties as assigned.
WORKING CONDITIONS:
- The HC Supervisor will work in the office one day per week and as needed for administrative responsibilities.
- This individual will provide direct care or transportation to clients as needed.
- Sit stand bend lift and pull intermittently.
- May also require spending time on location at a consumers home evenings weekends and holidays.
QUALIFICATIONS:
- 3 years of supervisory experience in DD field.
- High school diploma or equivalent.
- Extensive knowledge and experience in DODD compliance.
- Strong communication and organizational skills.
- Prior experience in residential care program management. Additional hands-on experience in a transportation Program would be a plus.
- Self-motivated with ability to work under little supervision.
- Ability to complete and pass a background check per company standards.
- Positive attitude with the desire to help others.
- 21 years or older.
- Valid drivers license with less than 6 BMV points and auto insurance.
