Are you passionate about empowering your community and connecting people to vital resources Do you thrive on building relationships and being a trusted advocate Were looking for a dynamic and dedicated Community Health Navigator II to join our team and make a tangible difference in peoples lives!
This grant-funded role is perfect for someone who is proactive community-focused and eager to help families navigate the healthcare and social service landscape. If youre ready to roll up your sleeves build bridges and be a catalyst for positive change we want to hear from you!
Responsibilities:
- Proactively engaging with community members and their families to share essential information and offer support acting as a friendly and knowledgeable guide.
- Advocating for community members ensuring their needs are heard and that they receive timely and appropriate access to care and services.
- Guiding individuals to a wide range of relevant resources from financial assistance programs and transportation services to essential health mental health and social services.
- Regularly participating in important public health meetings contributing your insights and expertise to broader community health initiatives.
Job Requirements
- Bachelors Degree in Public Health or other related field required
- Minimum of one (1) year of data entry experience required.
- Six (6) months of experience working with the public or with community groups.
- CA Drivers License required.
- Fire Safety (FIRESAFETY) within 90 days is required
