Bayside Health is a public health service delivering high-quality care across every stage of life for close to 1.2 million people living in metropolitan Melbourne the Mornington Peninsula Koo Wee Rup Bass Coast and Southern Gippsland. The Department
The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on quality of life and participation in care is a primary goal of the Department. The Social Work Service includes approximately 52 EFT (operational reports) with an additional 70 EFT (professional reports) who provide interventions to inpatients and outpatients across all Alfred Health clinical programs. The Social Work Service aligns to the Alfred Health strategic goals and operational performance targets. Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services that increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home.
The Position Summary
The Social Worker will be responsible for the provision of advanced social work services to patients and their families/significant others. This role will require an experienced Social worker with sound skills to support both Alfred Healths; General Medicine acute inpatient unit and Hospital in the Home (HITH) program. This role allows for social work to work uniquely in supporting patients across the continuum of care by supporting patients transition home after an admission to hospital. General Medicine at Alfred Health comprises of a team of doctors nurses and allied health professionals who provide acute care for a wide variety of conditions for patients within the metropolitan area and across the state. The ability to conduct home visits will be required for this role.
