Job Summary and Responsibilities
As a Navigator RN you will provide expert nursing guidance and support to patients as they navigate complex healthcare journeys. Every day you will assess patient needs educate on treatment options coordinate care across multiple providers and facilitate access to resources acting as a central point of contact. To be successful you will demonstrate strong clinical knowledge exceptional communication and problem-solving skills and a compassionate patient-centered approach to ensure seamless and informed care.
- Provides coordination of care and support for patients families and caregivers throughout the continuum of care.
- Provides education for patient caregiver and family on disease process plan of care treatment symptom management and survivorship.
- Effectively communicates with patient caregiver families and multidisciplinary healthcare teams.
- Provides nursing assessment through a process of gathering pertinent patients medical history and review of presenting symptoms.
- Demonstrates professionalism in the workplace with patients families healthcare team and community.
- Provide excellent customer service resulting in patient satisfaction with care.
Job Requirements
- Associates Degree in Nursing (ADN)
- 3 years of experience as a RN in an inpatient or outpatient setting
- Active Registered Nurse in the state of Arizona
Preferred
- Bachelors Degree in Nursing (BSN)
