Territory: Bloomington Full-Time Field-Based

What Youll Do

As a Registered Nurse in Home Health you will:

• Deliver skilled nursing care to patients in their homes

• Complete comprehensive patient assessments and develop individualized plans of care

• Serve as the primary nurse for your caseloadassessing planning coordinating implementing and evaluating care

• Perform initial evaluations routine visits and re-evaluations based on patient needs

• Monitor and document patient progress changes in condition and outcomes

• Communicate significant findings and care updates with physicians clinical leadership and the interdisciplinary team

• Revise care plans as patient needs evolve

• Ensure compliance with HIPAA and all regulatory standards

What Were Looking For

Required:

• Current unrestricted RN license in the state

• At least 1 year of clinical nursing experience

• Ability and willingness to travel within the Bloomington territory (approximately 50%)

• Valid drivers license reliable vehicle and current auto insurance

Preferred:

• Associates or Bachelors Degree in Nursing

• Previous Home Health or Hospice experience

• Experience with HomeCare HomeBase
