About the role

We are seeking a dynamic and skilled Care Finder to support older people to age well remain connected and maintain autonomy and quality of life. This role plays a critical part in helping older community members understand access and navigate aged care services and broader community supports.

You will work with older people experiencing complex and often intersecting challenges providing guidance advocacy and practical support while collaborating with other Councils and community organisations.

Come and join our friendly and committed Ageing Well team focused on empowering older people to live and age well in Merri-bek. Open to secondments internal and external applications.

Skills and experience require to perform the role

  • Experience working with older people and demonstrated ability to provide support to navigate the service system for older people experiencing homelessness mental health issues drug and alcohol issues elder and systems abuse neglect financial disadvantage or cognitive decline
  • Strong knowledge of aged care services and other community supports
  • Ability to manage complex cases with sensitivity and professionalism
  • Excellent communication advocacy and problem-solving skills
  • Ability to work independently and collaboratively
  • Demonstrated ability to engage with others bring empathy cultural awareness and diversity awareness in relation to older people and their families
