Responsibilities
Working as a member of the PACE clinic and Interdisciplinary team the PACE Pharmacist proactively and collaboratively identifies and coordinates the resolution of medication-related issues based on an interdisciplinary patient-specific plan of care. Working at the top of their license and taking a comprehensive approach the pharmacist helps participants achieve their best possible outcomes from their medication therapies. The pharmacist also serves as the medication expert to raise the overall knowledge of the Interdisciplinary team and improve workflows where possible.
Participant Care
Perform Comprehensive Medication Reviews for high risk participants.
Evaluates medication regimen of participants for efficacy and cost effectiveness.
Recognizes and reports drug-drug and drugfood interactions.
Monitors for inappropriate drug therapy.
Recognizes and reports allergies.
Recognizes and assesses medication compliance and noncompliance.
Monitors response to drug therapy as appropriate.
Patient safety monitoring and reporting; respond to health plan requirements for acetaminophen and narcotic abuse issues.
Prior authorization and approval overrides for issues with dispensing to ensure patient receives medication
Face to face evaluation with new participants and annual assessments.
Provide respiratory inhaler evaluation drug therapy and guidance for patients with COPD.
Adjust doses when allowed by Collaborative Practice Agreement or provide recommendations to prescribers.
Perform medication reconciliation to ensure patients medication list is as accurate as possible particularly during transitions of care.
Order appropriate lab tests to ensure adequate monitoring of medication therapies.
Provide recommendations to help patients manage their out of pocket costs for medications.
Provide patient education around medications devices and disease states.
Quality Improvement
Participate in projects to improve the medication use process at the clinic and health system in
general
Provide formal and informal training to other staff to improve their medication-related knowledge
Assist in developing and revising standards of care for the MTM Pharmacy team
Collaborate with clinic staff to improve workflows and address site-specific needs
Participate in appropriate meetings within the health system
Training/teaching for staff and participants.
Organizes and participates in the Pharmacy committee.
Participates in various Quality Improvement initiatives.
Qualifications
Education
PharmD
Experience
5 years experience as a practicing pharmacist OR pharmacy residency in geriatrics or ambulatory care
License(s)/Certification(s)
Valid drivers license when driving any vehicle for work-related reasons.
Current license to practice pharmacy in the state of Iowa.
Maintain current Mandatory Reporter
Knowledge/Skills/Abilities
