Referrals Coordinator The Referrals Coordinator is responsible for managing and facilitating patient referrals ensuring seamless communication between healthcare providers patients and internal teams. The Referrals Coordinator serves as a key point of contact to ensure timely efficient and patient-centered referral processes while supporting overall departmental goals and compliance standards.
Location: This role is located in Midtown - Sacramento CA (95811).
Essential Functions Process Referrals
- Coordinate the authorization and referral process for each assigned referral.
- Process referrals for requested services ensuring timely and thorough completion.
- Monitor referral request for completeness and take appropriate steps to review request and obtain additional information when requested services are incomplete or inconsistent with scope of service.
Documentation - Provide accurate data entry of referral or authorization information into the electronic medical record.
- Send complete information to patient insurance (when applicable) and referring specialist in order to coordinate referral.
- Consistently utilize the departments documentation protocols.
Customer Service - Demonstrate a caring and interested manner.
- Listen carefully to patients questions and concerns.
- Handle telephone calls in a friendly courteous and sensitive manner.
- Return calls and follows up in timely manner.
- Interacts collaboratively with clinical staff other ancillary departments and outside entities (specialist office insurances etc.).
- Resolves differences in a spirit of cooperation and creates solutions which benefits all parties especially clinic patients.
Referral Tracking and Follow up - Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety.
- Ensure the referral is completed according to the department guidelines and within the appropriate timeframe.
- Make outreach efforts to insurances patients specialist office and care team as set forth by department guideline and clinical priorities.
Minimum Requirements
- High School Diploma or equivalent
- 1-2 years of Referrals Management/care coordination experience
Preferred Background
- Medical assistant experience is preferred
